PHYSICAL INFORMATION FOR 2024 - 2025

For the 24-25’ school year, ALL athletes will be required to electronically submit their IHSAA Pre-participation Physical Examination, Pg. 3, and electronic Forms Concussion Sudden Cardiac Arrest form, Consent to Treat form, FERPA form, HIPPA form) to Rankonesport.com. Any physical form dated prior to 4/1/24 will not be counted as a valid physical for the school year 2024-2025 at Center Grove School Corporation. Physicals will not be accepted via Email or In-Person, in the Athletic Office or the Athletic Trainers Offices.

If a physical is marked as anything besides  “Cleared for all sports without Restrictions,” the physical will not be valid until a doctor's note is submitted and clears the athlete for the aforementioned reason.

**PHYSICALS ARE DUE BY JULY 8, 2024.**

1. Log into Rank One:

Go to www.rankone.com or the button on this page. (Desktop is advised.)

• Click the “Parents Click Here” tab 

• Click “Go To Forms” tab
• Click “Indiana” tab 
• Click “Center Grove” tab
• Click “Proceed to Online Forms” tab 
• Click “Continue as a guest” tab
• Enter last name and student ID number
• Click “Find Student” tab

2. Complete 4 Forms:

(Note: Parents/students will need to know Student ID number to complete forms online.)

Form 1: Concussion and Sudden Cardiac Arrest Acknowledgment

• Must first read “Heads Up Concussion” Fact Sheet for Athletes and Parents
• Must first read “Sudden Cardiac Arrest” Fact Sheet for Athletes and Parents. 

Form 2: Consent to Treat (contains emergency contact info) 
Form 3: HIPAA Authorization Form 
Form 4: FERPA Authorization Form

3.  Submitting Physical Examination (Pg. 3 of packet) on RankOne:

1.  Follow Steps 1-7 from above.
2.  Click "Physical Upload Form"
3.  Make sure first four boxes are accurate for your student (Name, ID, School attending in 24’-25’).
4.  Click "PDF or Picture," based on how your pg. 3 is saved on your device
5.  Click "Select"
6.  If you have your PDF or picture saved:
   • Click "Photo Library"
   • Click "PDF/Picture" of completed pg. 3
7.  If you don’t have your PDF or Picture saved:
   • Click "Take Photo or Video"
   • Take your picture of the completed Pg. 3. *Picture will appear as uploaded when there is a green indicator above the blank box adjacent to “Select.”*
8.  Click “I acknowledge that the school is not liable for medical expenses if an injury occurs” check box.
9.  Parent/Guardian prints their name and signs their signature.
10.  Enter today's date
11.  Click “I Agree” check box.

Once all forms are completed, all forms will be electronically filed.

2.  Download page 3, the completed PHYSICAL EXAMINATION page of the IHSAA Pre-participation Physical Form to Rank One.
3.  Fill out and sign page 5 of the IHSAA Pre-participation Physical Form on Rank One.
